Job Summary:
This is a professional position responsible for providing the technical assistance necessary to ensure a child is linked to services with community mental health and substance abuse programs and Department of Juvenile Justice (DJJ). This position will directly provide connections/support for children and adults with mental health, substance abuse and behavioral health needs.
Essential Functions:

Establish and maintain positive working relationships with Heartland for Children internal and external customers

Knowledgeable of child welfare and case management

Knowledgeable of mental health, utilization management and DJJ processes

Knowledgeable of Comprehensive Behavioral Health Assessments (CBHA), mental health referrals and mental health services

Receive and process CBHA referrals that are submitted by our partners within required timeframes.

Periodically review CBHAs for quality and provide feedback to providers when necessary.

Provide connections and support for mental health, substance and behavioral health needs of children and adults

Attend DJJ lockout staffings, coordinate meetings and recommend services for kids to help divert placement in licensed care

Participate in multi-disciplinary staffings to include but not limited to Crossover Youth Staffings, Youth at Risk Staffings, Local Review Team Staffings and other staffings as requested.

Identify trends, good practice and barriers related to these processes and make written recommendations for improvement and/or training opportunities

Perform additional duties as assigned
Qualifications:
Bachelor’s degree in social services, health care, business or related field required. Master’s degree preferred. Extensive experience in child welfare and/or child abuse prevention preferred.
