Case Management Support - Prevention & Diversion

Job Summary: 

This is a professional position responsible for providing administrative and clerical support to the prevention/diversion team.  This position will be responsible for facilitating transfer of dependency cases to ensure children achieve timely permanency.  This position will directly provide connections/support for children and adults with mental health, substance abuse and behavioral health needs.

Essential Functions:

  • Establish and maintain positive working relationships with internal and external customers

  • Complete pre-work to assess the readiness of case transfers, determine which providers and services can be implemented timely and to ensure the case management organizations assigned are prepared and supported in becoming the primary worker on the cases

  • Review case history to ensure all placement options for the youth have been explored and discussed

  • Participate in and/or lead case transfer staffings

  • Track and maintain case transfer tracking data

  • Create and submit monthly/annual case transfer reporting data to the manager

  • Provide connections and support for mental health, substance and behavioral health needs of children and adults

  • Provide monthly roll up reports for the department to submit to the program manager

  • Assist with coordinating calendars/schedules for the prevention/diversion staff

  • Assist the prevention/diversion team by submitting referrals and engaging providers

  • Review and maintain community services material and assist in identifying gaps in services

  • Create surveys, evaluations, and registration forms as needed for staffings and/or training

  • Identify and connect with providers who offer services to HFC clients

  • Collect receipts and organize credit card transactions to be submitted to the Chief Financial Officer

  • Develop a resource pool for speakers, venues and gather other community service-related materials

  • Serve as backup to provide onboarding training for new employees

  • Perform other duties and special projects as assigned

Required Qualifications: 

High School Diploma or GED equivalent required.  Bachelor’s degree in a related field preferred. One year experience in the child welfare system, training, education, and/or customer service experience is preferred.  Administrative organizational skills and computer literacy are essential.
