Case Management Support - Integrated Health

Job Summary: 

This is a professional administrative support position responsible for assisting the Integrated Health Team in the coordination of behavioral and mental health/substance abuse services for children through the Medicaid Managed Care Plan.

Essential Functions:

  • Establish and maintain positive working relationships with HFC customers

  • Perform routine administrative functions, drafting correspondence, scheduling appointments, organizing and maintaining electronic files

  • Assist case managers, foster parents, caregivers and medical offices with Medicaid inquiries for access to Medicaid services

  • Responsible for ensuring there is no lapse in coverage from notices provided by Agency for Health Care Administration for children with open child welfare cases

  • Notify HFC REVMAX Specialists with insurance reinstatements for youth

  • Review medical bills, research coverage and forward to appropriate designee

  • Complete necessary primary care physician changes to ensure children receive appropriate medical care

  • Utilize data systems (FSFN, FLMMIS, DOH Immunization Portal, PRO, JJIS) to review information, pull reports and enter updates when applicable  

  • Communicate with medical offices to coordinate timely appointments, as necessary

  • Coordinate distribution of all member ID cards, handbook or other notifications to case managers

  • Maintain relationships with insurance company representatives and provided necessary documents for change requests and provide assigned case manager information to Care Management teams     

  • Complete medicaid managed plan changes via the medicaid member portal or customer service line for caregivers and case management

  • Upload Multi-Disciplinary Team staffing notes, Suitability Assessment recommendations and medical records into data systems (FSFN and internal data systems)

  • Provides case manager assignment to Lakeland Regional Health and Peace River Baker Act facilities

  • Provide parents, caregivers and case managers with guidance and support when navigating Medicaid Plan Portals for creating profiles

  • Monitors phone calls and forwards to correct HFC team, CMO or CBC

  • Assists IH team by searching for medical providers

  • Orders department supplies

  • Perform special projects as assigned

Required Qualifications: 

High school diploma or GED required. Three years data entry experience in a mental health or related field preferred. Minimum two years data entry experience.
