
Family Connections & MDT Specialist

Job Summary:
This position involves conducting extensive searches to locate and engage relatives and non-relative for children in care. In addition, this position will be facilitating staffings to transition children in care from residential group care to less restrictive, family-centered placements. This is an autonomous, highly driven professional position to achieve permanency for children and families.
Essential Functions:

Establish and maintain positive working relationship with HFC customers

Utilize mapping and family-finding techniques to thoroughly search for and engage relatives and non-relatives for placement options

Meet with youth to build rapport, explore their history, and identify potential family, friends, and connections

Attend, participate in, and conduct family-finding group discussions/staffings with all involved parties

Maintain a family-finding tracking log to monitor tasks, progress, and follow-up with case management organizations

Regularly review and identify changes to Florida Statutes, DCF operational procedures, administrative rules, and policies, ensuring all parties are informed of updates and potential issues

Maintain confidentiality and open communication with life coaches and case management organizations

Coordinate MDT meetings/staffings involving DCF, case management organizations, GALs, parents, youth, caregivers, family members, community providers, and other involved parties to assess options and make decisions in the best interest of the child(ren)

Receive and review all incoming MDT referrals for placement

Maintain an organized tracking system for multidisciplinary referrals, meetings, and confirmation of required attendees

Provide timely staffing/meeting invitations and notifications to all participants

Provide supporting documentation to MDT participants

Ensure MDT participants understand confidentiality requirements, including HIPAA and statutory obligations

Assist participants in identifying family strengths when establishing goals for the child; ensure best-interest factors and statutory requirements for children age 3 and younger (per S. 39.4022, F.S.) are considered

Provide notification to Children’s Legal Services and DCF Regional staff within 48 hours when unanimous consensus cannot be reached

Facilitate consensus-building among MDT participants during meetings/staffings

Share staffing/meeting notes with the court and all participants

Develop written reports for the court outlining decisions made during MDT meetings and feedback from each participant.

Document MDT outcomes in the Florida Safe Families Network (FSFN)

Review Florida Statutes, DCF Operational Procedures, Administrative Rules, and policies; ensure participants are aware of confidentiality requirements and potential case-specific issues

Cross-train co-workers to ensure coverage for the MDT staffing process

Participate in ongoing trainings, team meetings, and planning sessions

Perform special projects as assigned
Required Qualifications:
Bachelor’s degree in social work or related field. Experience working with children/youth in foster care, knowledge of the child welfare system, customer service and child welfare certification preferred.
