Free Health Services: Polk HealthCare Plan and the Florida Department of Health partner to provide free health services to memebers of the community and provide Polk HealthCare Plan education and enrollment assistance.
Polk HealthCare Plan in the Community: A case manager from Polk HealthCare Plan will be available at the provided location to provide more information about the plan and to assist residents in applying for the plan.
