Heartland for Children (HFC) embraces an integrated Performance and Quality Improvement Program that seeks to involve all of our stakeholders, including you! Each week, HFC hosts a weekly Performance and Quality Improvement (PQI) Committee that involves stakeholders from HFC, the Department of Children and Families, Case Management Organizations, Children’s Legal Services, network service providers and other stakeholders. During these committee meetings, the improvement cycle illustrated below is utilized to enable the system of care to ensure the safety, permanency, and well-being of children in Polk, Hardee, and Highlands Counties.

Each week, HFC hosts a weekly Performance and Quality Improvement (PQI) Committee that involves stakeholders from HFC, the Department of Children and Families, Case Management Organizations, Children’s Legal Services, network service providers and other stakeholders. During these committee meetings, the improvement cycle illustrated below is utilized to enable the system of care to ensure the safety, permanency, and well-being of children in Polk, Hardee, and Highlands Counties. This cycle is simpler than it may look at first glance. The blue arrows represent the process improvement method commonly used within HFC’s PQI Committee and sub-committees (or Ad Hoc Committees). This process involves defining our goals (also known as performance measures), collecting data regarding these measures, analyzing the data to determine what may be preventing HFC from achieving the established targets, implementing solutions (also known as countermeasures) to improve performance, and ensuring that HFC keeps meeting its goals. The green arrows are examples of how HFC utilizes the improvement cycle, and the dotted lines represent the use of the Ad Hoc Committees to take a closer look at an identified issue.
